The Life Plan development is driven by the person, with input and participation of all members of the care-planning team. It is crucial that all members of the care-planning team are included, engaged and working together towards the ultimate outcome of a comprehensive person-centered service plan that meets the needs, overarching safeguards and life goals of the person.

It is the responsibility of the habilitation provider to share relevant and pertinent information with the Care Manager prior to, during, and/or after the Life Plan meeting to support development of a comprehensive, person-centered Life Plan by the Care Manager. The care-planning team establishes the timeframe in which all needed and/or requested information or documents are to be shared/distributed, as well as who else within the care-planning team should have this information.

I have met very few people who have a plan for their lives. Most are passive spectators, watching their lives unfold a day at a time. They may plan their careers, the building of a new home, or even a vacation. But it never occurs to them to plan their life. As a result, many end up discouraged and disillusioned, wondering where they went wrong.


About six years ago, Daniel Harkavy, CEO and Head Coach of Building Champions, helped me create my very first Life Plan. It was the first time I ever thought systematically about what outcomes I wanted to see in the major categories of my life.


This process of creating and regularly reviewing my plan has been transformational. As my family, friends, career, and other interests have grown, this document has kept me on track, ensuring that I keep everything in balance (more or less). When things get chaotic, it serves as a map, telling me where I am and how to get back on the path to my intended destination.

This is because we tend to get to specific life spots without meaning, aim, or direction. To avoid this haphazard lifestyle, you need to learn how to make a life plan.


For the most part, a life plan keeps you on track. It gives you the direction that you need to follow throughout the years. You always have your vision in front of you, so whatever you think of doing is coherent with your long-term plans.


Whatever your future plans are, they must satisfy your morals and beliefs. Thus, before planning anything, you need to identify your life values. Doing so will keep you from developing a life plan that clashes with your spirituality and mental stability. Being in line with your values is exceptionally crucial for a happy life.


Basically, you need to break down your goals into small actionable and practical milestones. For example, to get a house, you must save money first. Your plan should include all the ways to save more money and get a suitable place for yourself.


First, create a detailed, prioritized list of steps you must take to achieve your goal. Start with the very first step and end with the last one. Some steps will involve a series of other steps. Finding a good nutrition specialist may involve doing internet research, checking your current health plan for nutrition specialists, and talking to friends to see if they have specialists they can recommend to you.


This may seem like a lot of work upfront, but it makes your action plan seem less overwhelming and more manageable. Most importantly, it helps you determine the specific actions you need to take at each stage.


While you can use a paper calendar or planner, an online calendar may be a better option. You can use it to set due dates or reminders for when each step needs to be taken, and it can be shared with other people who need to be in the know (like your running buddy or your mentor).


Ultimately, it all comes down to you and your motivation to improve your life. If you want to be in a state of existence one day where you can be proud of your achievements based on your scales of judgment, a life plan is the way to go.

The State of Minnesota, in Minnesota Statutes, section 120B.125, requires all students to have a Personal Learning Plan (PLP) that includes academic scheduling, career exploration, career- and employment-related skills, community partnerships, college access, all forms of postsecondary education and training, and experiential learning opportunities. A PLP is a personalized plan for students to use with parents, school staff, and administrators. It can be used to plan, assess, and record student progress toward career and college readiness.

Remember when we were younger, when people asked us what we wanted to be when we grew up? It may seem silly to ask an eight-year-old what they hope to do with their life. Maybe this small talk was also a way to get us to think about what we wanted from life. Most of us confidently answered this question, revealing our hopes and dreams.


Life planning outlines your big dreams and goals. Life plans allow us to put our dreams and goals on paper rather than just in our heads. When we see our dreams and goals listed in a life plan, they can serve as important reminders of our purpose and why we do the work we do. This may also enable us to make decisions that are in tune with our dreams.


I am going to provide you with a list of categories in your life. You may find it beneficial to rate your satisfaction with these categories on a scale of 1-10. This activity may help you figure out what works well and what needs to be improved in your life. This is not an exhaustive list, and I invite you to incorporate any life categories that are relevant to you as you complete this activity.


Many of us try to answer those questions with personality assessments, skills inventories, and conversations with close friends and mentors. Those can be helpful starting points, but the biggest questions of life deserve an intentional, integrated process led by a master Guide.
